HyCoSy Test: What You Need to Know

A HyCoSy (Hysterosalpingo Contrast Sonography) test is a minimally invasive procedure used to check if the fallopian tubes are open (“patent”) using an ultra-sound scanner. It is an alternative to the traditional HSG (hysterosalpingography) test which uses Xrays.

How is a HyCoSy Test Done?

The test does not need any anaesthesia, it hurts not a lot more than a cervical smear.

First a thin catheter is inserted through the cervix into the uterus. If the uterine cavity needs to be checked for polyps etc we first do an “aqua scan”. Plain saline is injected and the ultra-sound scan checks the lining of the womb and any polyps or fibroids which get outlined by the saline.

A sono-contrast agent (“Sonovue” or it’s alternative) is then injected, and a transvaginal ultrasound is used to track its flow through the fallopian tubes. The entire procedure typically takes around 20 minutes.

Preparing for a HyCoSy Test

  • The test is typically done in the week following your periods (before your expected ovulation), so that we don’t disturb a chance early pregnancy. This precaution ofcourse does not apply if you are not with a male partner.
  • You may need to take a mild pain reliever an hour beforehand. A pregnancy test is sometimes required to ensure you’re not pregnant. We prescribe you “prophylactic” antibiotics to prevent infection.

Interpreting HyCoSy Results

If the contrast agent flows freely, the fallopian tubes are open. Blockages suggest tubal issues that may require further treatment or IVF.

The “aqua-scan” tells us if the uterine cavity is regular and if there is a polyp / fibroid disturbing the outline of the cavity.

Complications of a HyCoSy Test

Complications are rare but can include mild cramping, infection, or allergic reactions to the contrast agent. If severe pain, heavy bleeding, or fever occur, ring us immediately.
